Registration for the Fall 2019 Season is now open!

Join Queen Anne Soccer Club for a fantastic fall soccer season!
Our MOD program - U6 through U9 ages, starts the end of August and runs through the end of October.
ASSOCIATION - U10 through U18 ages, starts mid August and runs through mid November. There is the option for U9 & U10 teams to participate in the All City Jamboree in late November and for teams U11 and older to participate in the SYSA City Tournament, which runs from late November through mid December.

 

U6 - U9 (MOD)

 

 $110.00 Regular Registration fee runs fromMay 1st - June 30th

  

 $25.00 late fee for all registrations (U8 and U9 only) after June 30th.

 

 

 U10 - U18 (ASSOCIATION)

 

$170.00 Regular Registration fee runs fromMay 1st - June 30th

 

* $25.00 late fee for all registrations after June 30th.

We understand that plans can change, so cancellations are accepted until September 1st, 2019.

No refunds will be issued AFTER September 1st, unless we are unable to place your player on a team.

  Please register on time to ensure your player is able to be placed on a team for fall season.

 

When you register, your player will automatically be assigned to the age division that correlates to their date of birth.If you have an August Birthday and want to play up with your grade level, please contact our Registrar, Anne, at the email address provided below after receiving your Registration confirmation. We will attempt to honor these requests, however, these placements are dependent upon space and availability in each age group.

 

Birth Certificate or Passport Required during Registration
If you have registered previously with QASC, your player is already age verified. You will be able to continue through Registration without uploading proof of age again. If you are new to soccer in Queen Anne, you will need to upload proof of age to complete registration. ONLY a government issued ID is accepted.

 

If this is the first time your child or children are being registered, you are required by WA Youth Soccer to submit a copy of a certified (government issued) birth certificate or passport to use for age verification.

 

Your registration cannot be completed until your child/children have been age verified. Children cannot be assigned to teams until your registration is complete.

 

Medical Release Form

 

All SYSA Players are required to fill out a Medical Release and Waiver Form each year. Please have your Physician information available at time of registering.

If you have an existing account in Affinity, PLEASE DO NOT CREATE A NEW ACCOUNT. This will delay your ability to register your player. Please contact Anne for assistance. We can look up your username or assign a temporary password for you.
